Wellness Tier

$199/month

Includes our 24-hour health management and 24 hour CallBell (TM) services, weekly and as-needed phone wellness checks, and ONE monthly in-home RN wellness visit. Includes any On-Demand services that are necessary during that month's visit.

Health and Medication Tier

$259/month

Bi-weekly in-home RN visits for medication management and/or On-Demand nursing services. Includes all Wellness Tier services.

Medical Tier

$299/month

Weekly in-home RN visits for On-Demand nursing services and/or weekly medication management. Includes all Wellness Tier services.

On-Demand in-home RN visits. Services include:

In-home evaluation of acute or chronic health change with appropriate triage
Lab specimen collection and lab drop-off
Medication set-up and education
Disease education and instruction
Minor suture/staple removal

Overnight sleep analysis

$99

Includes analysis of one overnight sleep session, delivery and pick up of devices, and printed reports for you and your provider's records.

Credit card convenience fee of $4 applied to all monthly subscription charges when credit card used
