Absolutely! Otherwise known as Transitional Care, our team provides daily ongoing vital sign monitoring, in-home visits, and 24-hour RN availability for that critical early post-hospital discharge period. We provide the high contact health management you need to help you "transition" from the hospital to home, and keep you from having to go back to the hospital or ER.

Daily measurements include blood pressure, heart rate, respiratory rate, temperature, blood oxygen level, blood glucose, body weight, and any other metrics we deem necessary to get the most inclusive clinical picture of you day to day. What we measure will be tailored to your individual health concerns.
And remember...if we see something not quite right we're calling you to check in, so you can take readings anytime you want and our clinicians will review them for your peace of mind.
VitalWatch provides in-home RN visits at least once a month as part of our routine service.
In addition, we can provide additional home visits if your health status changes at any time. If you have an acute care need or sudden illness, or one of your chronic conditions flares up, VitalWatch can send one of our advanced practice nurses to provide a medical grade evaluation in your home. We'll collaborate with your provider to determine the best course of action for you.
Yes! In fact our service works as the perfect adjunct to your home health team. Home health can manage your skilled nursing needs while we help manage your chronic health concerns. Two teams working together for you!
In addition, once home health discharges you, VitalWatch can continue to provide health management on an ongoing basis. Should a new skilled nursing need arise, the VitalWatch team can notify your home health company to restart their services.
VitalWatch clinicians can generally collect blood specimens, urine specimens, nasal swabs (for Covid-19, influenza, RSV) and swabs for Strep throat. We will collect those specimens and bring them to your preferred lab for testing. These specimens can be collected during scheduled or On-Demand in-home visits.
